The FSK_LOV table is missing a record for the ADD event type. This causes the following issues within the Investigation UI:

1. When an alert has been added to a case, the Alert Details screen does not show anything in the Status column in the Alert Information field.
2. From the Alert Details screen of an alert that was added to a case, when you click Alert history the following error occurs:
   An unexpected error has occurred in the application. Please have an administrator review the system log. Details: java.lang.NullPointerException

   To resolve this problem run the following SQL code in a DBMS client session:

   insert into AMLCTR.fsk_lov values ('EVENT TYPE', 'ADD', 'BANK1KCTR', 'Alert added to a case', 28, 'US'); for each segment, where BANK1KCTR is a segment_id.
